A. Left upper back shave

B. Right upper back shave

Final Diagnosis

A. Atypical intraepidermal melanocytic proliferation, consistent with lentigo maligna, present in peripheral margin.

B. Nodular basal cell carcinoma, completely excised.

Microscopic Description

A. Sections reveal a melanocytic proliferation involving the epidermis. Atypical melanocytes with pagetoid spread are present. No invasion is identified. The lesion extends to the peripheral margin in the plane of section examined.

B. Examination of the specimen reveals a nodular basal cell carcinoma composed of basaloid cells arranged in nests with peripheral palisading and stromal clefting. No perineural invasion is identified.

Gross Description

A. Received in formalin is a tan-brown skin specimen labeled 'A. Left upper back shave' measuring 0.8 x 0.5 cm. There is a variegated pink patch 0.6 cm in greatest dimension. The specimen is submitted in its entirety. GA.

B. Received in formalin is a pink-tan skin specimen labeled 'B. Right upper back shave' measuring 0.6 x 0.4 cm. The specimen is inked at the margins and submitted in its entirety. GA.

Clinical History

A. 7 x 5 mm pigmented lesion left upper back, ? atypia ? SK.

B. 3 x 3 mm amelanotic lesion right upper back, ? BCC.

Final Diagnosis

No evidence of residual melanoma. Cicatrix.

Microscopic Description

Microscopic examination reveals scar tissue with fibrosis and focal chronic inflammation. No residual atypical melanocytic proliferation or atypia is identified. Areas previously harboring lentigo maligna are replaced by cicatricial changes, and all margins are free of tumor.

Gross Description

Received in formalin labeled 'A. Left upper back excision' is an elliptical specimen of skin measuring 2.5 cm in length and 1.5 cm in width. The specimen includes an area of prior biopsy scar. The tissue is inked at the margins. GA.
